About the Role
As an Admissions Registered Nurse (RN) for Hospice, you will find your passion and purpose by reimagining your career in a field dedicated to exceptional care. This role is designed for those who thrive in a supportive environment and are committed to providing comprehensive assessments and individualized plans of care for patients and families.
You will serve as a vital link within the interdisciplinary team, collaborating with patients, families, and physicians to coordinate care needs and ensure timely interventions. Your day will involve conducting admissions, managing care schedules, and providing essential education to caregivers. This position covers the UCONN, Waterbury, and Griffin Hospitals areas, specifically focusing on weekend General Inpatient (GIP) admissions and visits.
