Find your passion and purpose as a Hospice Admissions Registered Nurse. In this part-time role, you will join a team that cares deeply for patients and each other, committed to consistently providing exceptional care. You will serve the UCONN, Waterbury, and Griffin Hospitals coverage area, focusing on weekend General Inpatient (GIP) admissions and visits.
Your day will involve conducting comprehensive assessments and collaborating with patients, families, physicians, and the interdisciplinary team to develop and adjust individualized plans of care. You will coordinate care needs with internal and external partners, provide patient and caregiver education, and maintain accurate clinical records. As part of the team, you will participate in on-call rotations and manage care using the Case Management model.

Candidates should have previous nursing experience in hospice, home health, or general medical-surgical settings, along with a reliable vehicle and valid driver's license.
