About the Role
CenterWell Home Health is seeking a Dispensing Pharmacist to support safe, accurate medication management for patients receiving in-home care. This role is vital to our mission of enhancing patient autonomy and quality of life by ensuring that every medication regimen is optimized for the home setting.
In this position, you will serve as a critical link between prescribers, patients, and the broader care team. Your day will involve verifying prescriptions, reviewing complex medication profiles, and identifying potential interactions before they become issues. Beyond clinical verification, you will provide compassionate counseling to patients and caregivers, guiding them on proper use, adherence, and the safe storage and disposal of medications. You will collaborate closely with nurses, therapists, and physicians to integrate medication therapy into individualized care plans, ensuring seamless transitions of care and continuous clinical support.

Requirements
- verifiedActive and unrestricted Pharmacist license (RPh or PharmD) in the state of practice
