The Director of Care Management is a pivotal leadership position under the supervision of the North Division Executive Director of Care Management. This role is responsible for orchestrating and optimizing care management across St. Peter Hospital (390 beds) and Centralia Hospital (128 beds). You will lead and inspire a dedicated team to ensure a seamless continuum of care, patient advocacy, and efficient resource management from admission through to discharge. Integral to the organization's financial viability, you will oversee care management teams to coordinate the best next site of care for patients, ensuring consistent clinical outcomes while minimizing fragmentation within the healthcare delivery system.
This position serves as a master coordinator and communication facilitator, fostering an environment of collaboration to enhance patient care. You will champion cultural and religious sensitivity, ensuring that care respects diverse beliefs and backgrounds while building strong cooperative relationships with physicians and external agencies. The role offers a dynamic environment where you can leverage your autonomy to innovate and excel, contributing to meaningful change in healthcare delivery.
