Kindred Hospital Dayton is a 97-bed facility located downtown near the I-75 and I-35 junction, west of the Great Miami River. We specialize in extended recovery care, offering 51 Long-Term Acute Care Hospital (LTACH) beds that provide in-depth treatment similar to a traditional hospital but tailored for longer recovery periods.
Our 46-bed Behavioral Health Unit (BHU) serves adults ages 18+, including dedicated services for geriatric patients. We provide 24-hour supervised psychiatric treatment and crisis support for mental health issues. Like our LTACH, we partner closely with your physician to offer 24-hour clinical care seven days a week, ensuring you can begin your journey to wellness immediately.
This position serves as a vital link in the continuum of care, coordinating and facilitating patient management through effective collaboration with the Interdisciplinary Care Transitions (ICT) team. You will follow patients throughout their entire stay, ensuring the optimum utilization of resources and strict compliance with external review agencies.
Your day-to-day involves providing ongoing support and expertise through comprehensive assessment, care planning, and evaluation of individual patient needs. By integrating case management, utilization review, and discharge planning, you will enhance the quality of patient management and satisfaction while promoting cost-effectiveness and continuity of care. You will act as a patient advocate, helping patients and families make informed decisions and connecting high-risk individuals with additional support services.
