About the Role
As a Direct Support Associate, you will be the heart of our care delivery, providing essential support and supervision to individuals in home and community settings. This role exists to bridge the gap between clinical needs and daily living, helping individuals build independent living and social skills aligned with their personal care plans.
Your day will involve a dynamic mix of personal care assistance, meal preparation, and household tasks, all performed with a focus on dignity. You will accompany individuals to medical appointments, community outings, and recreational activities, ensuring they are safe and engaged. A key part of your responsibility is documenting care, monitoring behavior and health status, and collaborating closely with nurses and case managers to maintain coordinated, patient-centered care. You will work within a safe, respectful environment that strictly adheres to regulatory and infection-control standards.
