About the Role

As a Direct Care Worker, you will be the vital link between our clients and the care they need. You will partner with patients and their families to deliver compassionate, high-level care in the comfort of their own homes. This role exists to ensure that every client receives personalized attention that respects their autonomy and dignity.

In this position, you will perform or assist with personal care activities, home maintenance, and safe transfer techniques as prescribed in the plan of care. You will be responsible for documenting care provided and responding to the needs of the consumer and family in a timely manner. Your work will directly impact the well-being of those we serve, requiring you to adhere to strict infection control precautions and HIPAA guidelines while maintaining a professional and ethical demeanor.
