About the Role
This hybrid position serves as a bridge between in-clinic care and the digital health landscape. As a Digital Health Specialist, you will work 3 days in person at our Winter Haven clinic and 2 days remotely. You will provide digital health services, including virtual care and remote therapeutic monitoring, while participating in the development of the digital health program.
In this role, you will serve as a vital liaison between patients and in-clinic treating providers. You will develop and deliver personalized plans of care for a diverse patient population, utilizing cutting-edge software to objectively measure outcomes. Your day-to-day involves performing telehealth and Remote Therapeutic Monitoring (RTM) services, including interactive communication and monitoring, while efficiently documenting evaluations, treatments, and discharge notes. You will also work closely with operations to meet patient care needs and collaborate with the Director of Digital Health to optimize and grow remote services.
Hiring Process
Candidates must be board-licensed Physical Therapists with experience in the outpatient setting. We are looking for individuals dedicated to providing exceptional quality of care. Successful candidates must be willing to obtain licensure in other states to support our expanding digital services.
