About the Role
As a Digital Health Specialist, you will ignite your career by bridging the gap between in-clinic care and the digital world. This hybrid role is designed for a licensed Physical Therapist who thrives in a dynamic environment, splitting their time between our clinic and remote work. You will serve as a vital liaison between patients and in-clinic treating providers, ensuring seamless continuity of care.
In this position, you will participate in the development of our digital health program, delivering virtual care and remote therapeutic monitoring. You will work closely with the Director of Digital Health to optimize and grow remote services, utilizing cutting-edge software to objectively measure patient outcomes. Your day will involve developing personalized plans of care for a diverse patient population, performing telehealth visits, and analyzing regional service metrics to drive program success.
