About the Role
This hybrid position serves as a Digital Health Specialist, bridging the gap between in-clinic treating providers and patients through digital health services. You will act as a liaison between patients and the clinic, participating in the development of the digital health program while delivering virtual care and remote therapeutic monitoring.\n In this role, you will make a powerful impact on your local community by delivering inclusive physical therapy treatment to a diverse patient population with both orthopedic and neurologic diagnoses. You will develop and deliver personalized plans of care, objectively measure patient outcomes using cutting-edge software, and efficiently document evaluations, treatments, and discharge notes. A key part of your day involves using the digital platform to deliver telehealth visits, including assessments and treatment plan development, as well as performing Remote Therapeutic Monitoring (RTM) services.
You will work closely with operations to meet patient care and staffing needs, communicating with primary treating clinicians to revise treatment plans as appropriate. Additionally, you will maintain regular reporting and data analysis of regional service metrics and participate in program development and marketing efforts directed by the Director of Digital Health.
Work Model
This is a hybrid role requiring you to work 3 days in person at a CORA clinic and 2 days working remotely. The position is full-time (40 hours/week, 5 days/week). Candidates must be licensed in any compact state and be willing to obtain licensure in other states as needed.
