About the Role
As a Digital Health Specialist, you will ignite your career by bridging the gap between in-clinic care and the digital world. This hybrid role is designed for a licensed Physical Therapist who thrives in a dynamic environment, splitting their time between our clinic and remote work. You will serve as a vital liaison between patients and in-clinic treating providers, ensuring seamless continuity of care.
In this position, you will participate in the development of our digital health program, delivering virtual care and remote therapeutic monitoring. You will develop personalized plans of care for a diverse patient population, utilizing cutting-edge software to objectively measure outcomes. Your day will involve conducting telehealth evaluations, assessments, and treatment planning while efficiently documenting patient progress. By working closely with operations and the Director of Digital Health, you will help optimize and grow our remote services to meet patient care needs.
Hiring Process
To join our team, candidates must hold a diploma from a CAPTE-approved Physical Therapy Program and possess a valid Physical Therapy state license (or be in the process of obtaining one).
