This position provides direct patient care within the In-hospital coverage department. You will see hospitalized patients via referral from licensed medical providers (RN, MD, PA, NP) who have a diagnosis of diabetes or are pre-diabetic and require counseling. Your role involves teaching patients about insulin, its delivery, and other prescribed oral agents. You will also instruct patients on survival skills and guide them to seek outpatient programs for additional education.
You will promote patient and family-centered care in a healing environment, educating patients and families on managing illness, post-treatment home care needs, and medication administration. This is a safety-sensitive position requiring the ability to work in a constant state of alertness. You will develop, implement, and document individual plans of care in collaboration with the interprofessional team, proactively ensuring communication of the plan across the continuum of care.
This is a PRN position with a 7:30 AM to 4:00 PM shift, Monday through Friday. The schedule operates on a 2-shift rotation within a 6-week cycle.
