As a Coordinator of Community Services, you will serve as a vital advocate and resource navigator for individuals in our care. This role exists to bridge the gap between an individual's aspirations and the services they need to achieve them. You will work closely with the people we support and their teams to develop and monitor Person-Centered Plans (PCPs), ensuring that every step taken aligns with their personal vision.
Your day will involve a dynamic mix of coordination, advocacy, and relationship building. You will manage a caseload of 25–30 individuals, facilitating team meetings and encouraging participants to take the lead in their own planning processes. A significant part of your work will be resolving conflicts, negotiating resources, and exploring alternative funding sources to meet identified needs. You will also conduct regular visits to individuals in their settings—at least three times annually—to monitor progress and ensure the continuity of supports. Accuracy and timeliness are paramount, as you will document all activities in the State Database within 24 hours to meet billing and compliance standards.
