This position serves as a vital bridge between clinical care and community living within the Care Link BH Home department. The role is dedicated to coordinating services and resources that promote rehabilitation, recovery, and resiliency for individuals and families facing serious emotional, neurobiological, or behavioral disorders. Whether supporting children at risk, adults with severe mental illness, or individuals with co-occurring disorders, the focus is on identifying strengths to overcome barriers and fostering independent functioning.
In this capacity, you will act as a patient advocate and mentor, helping clients navigate complex healthcare systems, access community resources, and manage financial assistance programs. The role involves facilitating the development of essential life skills, from daily living activities to crisis management, ensuring that patients can thrive in home, school, work, and social environments. You will collaborate closely with multidisciplinary teams to create personalized service plans that include recovery strategies, crisis interventions, and advanced directives.
Working within the Behavioral and Mental Health department, you will be part of a team committed to the core expectations of the Patient Centered Medical Home. The environment prioritizes proactive patient safety, cultural competence, and the elimination of access barriers. The role requires a high degree of collaboration with medical providers, community resource partners, and the judicial system to ensure comprehensive care for a diverse patient population ranging from neonates to geriatric adults.
Candidates must possess a Bachelor's Degree in a related discipline and at least one year of directly related experience. Essential credentials include a valid New Mexico Driver's License and CPR/BLS certification (to be obtained within 30 days). A UNM Vehicle Operator's Permit is required within 60 days. While bilingual proficiency in Spanish, Keres, Tewa, Tiwa, Towa, Zuni, or Navajo is nonessential, it is highly valued for serving our diverse community.
