Communicare is a community mental health center located in North Central Mississippi, serving Calhoun, Lafayette, Marshall, Panola, Tate, and Yalobusha counties. As the people who care, our mission is to provide specialized treatment services for adults with serious mental illness, children with serious emotional disturbances, individuals with substance abuse disorders, and those with intellectual and developmental disabilities.
As a Community Support Specialist, you will be a vital link in our continuum of care, providing case management and supportive services to clients with complex needs. This role exists to ensure that individuals in our community have access to the resources they need to thrive, from securing housing and medications to navigating the benefits system. You will take full ownership of your certification process and productivity goals, collaborating closely with the office manager and CSS Director to maintain high standards of care. Your day will involve conducting ongoing client assessments, developing treatment plans, and coordinating services across multiple agencies. You will provide supportive counseling to clients and their families, advocate for their needs, and conduct home visits as necessary. Additionally, you will play a key role in loss prevention by verifying active Medicaid and ensuring proper prior authorizations are in place before services are provided.
