This position serves as a dedicated Community Support Services (CSS) Case Manager, focused on providing comprehensive care to individuals with severe mental illness. The role exists to bridge the gap between clinical treatment and daily living, ensuring clients receive coordinated, holistic support within their communities.
In this capacity, you will manage a caseload of clients, conducting ongoing assessments to develop personalized treatment plans. Your day-to-day involves a blend of direct client interaction and administrative coordination. You will provide supportive counseling to clients and their families, assist with critical needs such as housing applications, benefit enrollment, and medication management, and advocate for clients to secure necessary mental health and community resources.
The role requires active fieldwork, including home visits tailored to each client's level of need. You will also serve as a liaison between the mental health center and external agencies, ensuring seamless service delivery. Additionally, you will play a key role in grant compliance and funding continuity, collaborating with area hospitals and the AMAP team to meet program objectives.
