The Community Health Worker (CHW) serves as a vital bridge between medical providers, primary care teams, and social services agencies. In this role, you will provide short-term care coordination designed to improve patient health and general well-being. Your work extends beyond the clinic walls, involving community outreach such as home visits, health screenings, and events to ensure patients receive the support they need.
You will act as an advocate and educator, helping patients navigate socio-economic barriers that impact their health. By connecting individuals to essential resources—including housing, food, mental health services, and insurance enrollment—you will facilitate a holistic approach to care. This position is integral to The Wright Center's mission, requiring you to embody our Core Values while working collaboratively with care managers, medical residents, and office staff to reduce cultural and systemic barriers.
The Wright Center operates as a Federally Qualified Healthcare Center Look-Alike (FQLA) and a Patient Centered Medical Home (PCMH), recognized by the National Committee for Quality Assurance (NCQA) and participating in an Accountable Care Organization (ACO). We are dedicated to serving a diverse community, and your role is central to our commitment to equity and access. You will represent the enterprise professionally, ensuring that every interaction reflects our dedication to patient-centered care and community engagement.
Candidates selected for an interview will be evaluated based on their alignment with our mission and their ability to work effectively within the EOS® model. The process includes a review of qualifications and a discussion regarding your experience in community health and care coordination.
