About the Role
As a Community Health RN, you will serve as a vital member of an interdisciplinary care team within a NORC (Naturally Occurring Retirement Community) or senior living community. Your mission is to coordinate a broad range of health and social services to support older adults in aging safely in place.
NORCs achieve this by facilitating and integrating existing community-based health and social services, while identifying and organizing additional supports needed to help residents remain independent and engaged in their homes and communities. In this role, you will collaborate with your team to conduct client assessments, develop individualized nursing care plans, and provide health promotion education. You will also act as an advocate, facilitating referrals and linkages to appropriate healthcare services based on client needs.

Requirements
- verifiedRegistered Professional Nurse license in New York
- verifiedCPR/BLS Certification
- verifiedAssociate's degree in nursing from approved program
- verifiedMinimum of two years' clinical home care experience
