Under the general supervision of the PCMH Manager, the Community Health Nurse provides comprehensive home visit nursing services through a referral-based process. This role is designed to address the clinical and psychosocial needs of referred patients and families by collaborating with the Care Coordination and Case Management multidisciplinary team.
The position focuses primarily on direct patient care, chronic disease support, patient education, and the coordination of services in home and community settings. The incumbent operates within established nursing standards, policies, and physician standing orders, supporting program goals while maintaining the principles of WIHCC's Vision, Mission, and Value Statements. The role requires a high degree of cultural awareness and sensitivity, particularly in serving diverse communities.
Work is performed in a variety of settings, including office environments and outdoor locations. The role frequently involves travel to patient homes, including in inclement weather or high-risk neighborhoods. The environment may involve exposure to physical risks such as bloodborne pathogens or hazardous chemicals, requiring strict adherence to safety protocols. While the position is primarily field-based, it also includes participation in community health fairs and emergency response activities. Required work schedules may include evening, weekend, overnight shifts, or irregular hours as operational needs dictate.
Applicants must successfully pass a background investigation. This position is classified as a Child Care position, subject to the requirements of the Indian Child Protection and Family Violence Prevention Act (ICPFVP Act). A satisfactory background check is mandatory for all candidates.
