VNS Health has been committed to meeting the needs of New Yorkers for over 130 years. As one of the largest nonprofit home- and community-based health care organizations in the country, we bring together more than 11,500 team members to make a difference in the lives of over 99,000 patients and members on any given day.
This position ensures the quality, safe delivery, and cost-effective home care services while coordinating care that reflects VNS Health's philosophy. You will serve as a clinical resource and subject matter expert, working under general direction to maintain high standards of care across the organization.
In this role, you will collaborate with personnel from other provider organizations to coordinate patient services and develop joint educational initiatives. Your day-to-day responsibilities include reviewing clinical documentation for accuracy and compliance, developing clinical policies, and participating in the creation of evaluation tools for Registered Nurse Field Supervisors. You will also support business growth by contributing to marketing strategies and planning nursing service opportunities.
You will act as a vital link between leadership and the field, ensuring timely supervision, assessment, and crisis management. This involves counseling individuals regarding appropriate levels of care, monitoring patient conditions for necessary escalations, and investigating incidents or complaints. Additionally, you will support department leadership in onboarding new staff and conducting field supervision for RNFS during their initial months and annually thereafter.
Candidates are encouraged to apply directly through our career portal. The selection process typically involves a review of qualifications followed by interviews with department leadership to assess clinical expertise and cultural fit.
