About the Role
As a Clinical Care Nurse, you will be the vital link between medical expertise and personal care. This role exists to provide comprehensive, in-home nursing services that go beyond standard treatment; it is about building meaningful relationships with patients and their families. You will conduct thorough assessments, develop individualized care plans, and administer complex treatments such as wound care, IV therapy, and medication management.
Your day will involve collaborating closely with an interdisciplinary team of physicians, therapists, and social workers to ensure seamless, patient-centered care. You will document all interventions in our Electronic Health Record (EHR) system while upholding strict regulatory standards. Beyond clinical duties, you will serve as an educator, empowering patients and caregivers with the knowledge needed to manage conditions safely at home. Requirements
- verifiedActive RN license in practicing state
- verifiedBLS (Basic Life Support) certification
- verifiedValid driver's license and reliable transportation
Nice to Have
ACLS certification. Home health or community nursing experience.
