About the Role

As a Home Health Registered Nurse, you will be the cornerstone of a patient's care journey, conducting comprehensive assessments and developing individualized care plans tailored to their specific conditions. This role exists to bridge the gap between clinical expertise and the comfort of home, ensuring that patients receive the highest standard of care while maintaining their autonomy.

In your day-to-day, you will administer medications, IV therapy, and wound care per physician orders, while closely monitoring patient progress. You will serve as an educator, guiding patients and their families on disease management, medication adherence, and home safety. Beyond clinical duties, you will document visits accurately in EMR/OASIS systems to ensure regulatory compliance and coordinate seamlessly with physicians, therapists, and other interdisciplinary team members. You will also participate in an on-call rotation to respond to urgent patient needs, supporting quality improvement initiatives and mentoring teammates to uphold CenterWell standards.
