The Certified Recovery Support Specialist (CRSS) serves as a vital bridge between clinical care and community living, providing experience, education, and professional services to assist individuals in developing and maintaining recovery-oriented, wellness-focused lifestyles. This role exists to ensure patients receive comprehensive support that honors their inherent dignity, individuality, culture, and beliefs.
In this position, you will engage patients within the living room process, establishing collaborative relationships to provide practical and personalized support for entering and adhering to discharge planning. Your day will involve identifying patient needs and utilizing evidence-based approaches to navigate stages of change, while also providing advocacy, education, and therapeutic intervention for those in crisis. You will facilitate warm handoffs, ensuring patients are engaged by intake staff and remain connected to treatment over time. Additionally, you will work to bridge care with insurance-correlated community services, assist with transportation barriers, and provide follow-up instructions to ensure continuity of care after discharge.
