This role is part of the UPC Peer Support department within Behavioral and Mental Health. The team is dedicated to promoting rehabilitation, recovery, and resiliency for individuals and families facing serious emotional, neurobiological, and behavioral disorders. The mission centers on identifying strengths to aid in recovery while addressing barriers to independent community functioning.
As a Certified Peer Specialist, you will coordinate services and resources to support patients across the lifespan, from neonates to geriatric adults. Your work involves providing personalized interventions to help individuals make informed, independent decisions, particularly during crisis situations. You will serve as a mentor to instill hope, teach symptom management and relapse prevention skills, and assist clients in navigating the healthcare system to access necessary medical, rehabilitative, and financial resources.
The position requires a proactive approach to patient safety and collaboration with multidisciplinary teams to eliminate barriers to culturally competent care. You will facilitate the development of life skills, including daily living activities, socialization, and community functioning, while assisting in the creation of comprehensive service plans that include crisis management and recovery goals.
