Certified Case Manager

About the Role

The Certified Case Manager (CCM) serves as a key member of the interdisciplinary team, actively managing and directing resource utilization to achieve the highest quality outcomes during a patient's rehabilitation experience. This role coordinates and advocates for patients throughout their hospitalization, from admission through post-discharge. As an effective communicator, the CCM oversees healthcare delivery, facilitates interdisciplinary plan of care decisions, and ensures timely communication regarding patient care.

The CCM establishes and monitors discharge plan implementation while identifying and addressing psychosocial and support system issues. Responsibilities include overseeing the coordination of services, managing admission and discharge logistics, leading team conferences, providing patient and family education, handling payor relations, and ensuring total fiscal management. The CCM also performs ongoing utilization reviews, acts as a liaison to payors to assure cost-effective treatment, and ensures all regulations regarding patient rights are fulfilled.
