This position serves as a critical bridge between clinical expertise and community-based care, conducting comprehensive assessments for UAS-NY members to determine their eligibility and care needs. The role exists to ensure that both potential and existing members receive safe, high-quality, and cost-effective health services that allow them to remain in their communities. You will work under general supervision to evaluate clinical, environmental, and social conditions, translating these findings into individualized plans of care.
In this role, you will navigate a dynamic landscape of in-home and telehealth interactions across upstate and downstate counties. Your day involves utilizing clinical skills to document long-term needs, interviewing members and families, and collaborating with providers to identify alternative care options. You will act as an advocate, explaining VNS Health Plan benefits and navigating complex regulatory landscapes to secure necessary services. Success in this position requires balancing strong relationship management with the ability to negotiate "win-win" solutions when overcoming barriers to care.
Candidates will be evaluated based on their clinical background, specifically their experience in assessment, homecare, or hospital settings. The process includes a review of qualifications, including a current New York State Registered Professional Nurse license, and an assessment of interpersonal and conflict management skills. Preferred candidates will hold a Master's Degree in Nursing and possess Certified Case Manager status.
