About the Role
This position serves as a critical bridge between clinical expertise and community-based care, conducting comprehensive assessments to ensure members remain safe and supported within their homes. You will evaluate the clinical, environmental, and social conditions of both potential and existing members to establish individualized plans of care that promote high-quality, cost-effective health services.
In this role, you will navigate a dynamic day-to-day environment that involves conducting face-to-face or telehealth assessments across upstate and downstate counties. Your work requires close collaboration with members, families, and providers to document long-term needs and determine appropriate service plans. You will act as a clinical decision-maker, rendering determinations in accordance with VNS Health Plans policies and applicable state and federal regulations while managing service requests from various stakeholders.
Hiring Process

Key Responsibilities
- check_circleConduct face-to-face or telehealth UAS-NY assessments according to state guidelines and protocols
- check_circleAssess and document long-term community-based needs of potential and existing members
- check_circleDevelop individual plans of care to maintain member safety in the community
- check_circlePerform in-home assessments for members with significant condition changes
- check_circleCoordinate with Care Management, Legal, Compliance, and other departments as needed
- check_circleManage service requests from providers and members while rendering clinical determinations
- check_circleExplain VNS Health Plan benefits and member handbook details to clients
- check_circleIdentify alternative care options and contribute to safe, member-centered service plans
Requirements
- verifiedCurrent license to practice as a Registered Professional Nurse in New York State
- verifiedBachelor's Degree in nursing or equivalent work experience
- verifiedMinimum two years of clinical assessment, homecare or hospital experience
- verifiedWorking knowledge of Microsoft Excel, Power-Point, and Word
- verifiedKnowledge of Medicaid and/or Medicare regulations
Nice to Have
Certified Case Manager. Master's Degree in nursing or equivalent work experience. Working knowledge of UAS-NY.
