About the Role
As the Director of Case Management, you will serve as a key member of our leadership team, combining fulfilling career opportunities with the chance to make a meaningful difference in the well-being of your local community. This role is designed for an experienced and compassionate healthcare professional who seeks to align their expertise with personal values.
In this position, you will lead a team dedicated to the day-to-day operations and management of the Case Management department. You will oversee the interdisciplinary plan of care and discharge planning process, ensuring the effectiveness of services with a central focus on census management, patient care outcomes, and key care indicators. Beyond operational oversight, you will act as a patient and family advocate, ensuring services meet the unique needs of those we serve. You will also build strategic relationships with insurance companies, self-insured employers, and other healthcare networks while providing guidance, training, and support to Case Managers and staff.

Requirements
