About the Role

The Inpatient Social Worker at Sinai Hospital serves as a vital partner to the clinical team and medical providers, coordinating and implementing safe discharge plans for patients. This role exists to improve patient well-being and outcomes while ensuring healthcare services are utilized efficiently and timely. You will work within a team that values innovation and outcomes, delivering life-saving care to some of the most vulnerable patients in our community.

In this position, you will conduct initial and ongoing assessments to determine patient needs, developing focused discharge plans particularly for high-risk cases. You will collaborate closely with the clinical team to execute these plans, staying current with healthcare trends, regulations, and payer requirements to optimize patient care. The role offers a culture of collaboration with resources like unit-based practice councils and advanced clinical education support, allowing you to work at the top of your license.

Work Schedule

This position is scheduled for three 12-hour shifts per week, running from 7:00 am to 7:30 pm. Weekend commitment is required.
