About the Role
As a Case Manager Social Worker (LBSW) at Grace Medical Center, you will provide leadership to the healthcare team by coordinating post-acute care services and delivering psychosocial support to patients and their families. This role exists to ensure the timely utilization of acute care services while addressing the social, cultural, emotional, and spiritual needs of those we serve.
In this position, you will systematically gather information to identify patient needs and develop comprehensive plans of intervention. You will participate in interdisciplinary and inter-agency collaborative efforts to modify post-acute care strategies, ensuring that discharge plans are both efficient and compassionate. Your day will involve organizing discharge rounds, documenting assessments, and acting as a resource for the team on bed utilization and length of stay. You will also provide crisis intervention and counseling to assist patients and families with their emotional adjustment to medical episodes, while actively engaging in departmental improvement projects and shared governance committees.
