MHSU Care Manager (Mobile-NC)

About the Role

This is a mobile position based in Catawba, Iredell, and Rutherford counties, North Carolina. The Mental Health Substance Use Care Manager serves as a vital link between community hospitals, providers, and stakeholders to engage adults and children/adolescents in essential mental health and substance use services.

The role exists to provide proactive intervention and comprehensive care management, ensuring members receive appropriate assessment, oversight, and support. You will work in a variety of locations, conducting in-person visits and collaborating with families, caregivers, and community resources to bridge gaps in care and facilitate smooth transitions between clinical settings.

Day in the Life

Your work will involve conducting comprehensive assessments and developing tailored care plans for members. You will lead care team meetings, coordinating with Child/Adult High Risk Teams, DSS, school systems, and other community partners to address complex needs. A significant portion of your time will be spent monitoring progress, identifying service gaps, and connecting members to physical health, behavioral health, and social support services. Success in this role requires the ability to make independent, fact-based decisions while maintaining high levels of diplomacy and discretion in sensitive situations.
