About the Role
As a Social Worker, you will provide essential psychosocial services and support to ensure patients and their families receive care aligned with the Four Seasons Compassion for Life mission. You will operate across a variety of locations, including the Inpatient Unit (IPU) and community settings, adapting to the specific needs of each patient.
In this role, you will serve as a vital liaison between patients, families, and community health, welfare, and social agencies. Your day-to-day involves conducting holistic psychosocial assessments, developing and evaluating individualized plans of care, and facilitating discharge planning. You will also guide families through grief and loss, assist with financial advocacy, and support them in making final arrangements. The position requires active participation in interdisciplinary team meetings and the ability to function effectively in diverse care environments, from home settings to skilled nursing facilities.
