About the Role
This position serves as a vital link in the continuum of care, coordinating and facilitating patient management through effective collaboration with the Interdisciplinary Care Transitions (ICT) team. You will follow patients throughout their entire stay, ensuring the optimum utilization of resources and strict compliance with external review agencies.
Your day-to-day involves providing ongoing support and expertise through comprehensive assessment, care planning, implementation, and evaluation of individual patient needs. By integrating case management, utilization review, and discharge planning, you will enhance the quality of patient management and satisfaction while promoting continuity of care and cost-effectiveness. As a patient advocate, you will work to maximize the patient's and family's ability to make informed decisions and collaborate with clinical staff to achieve treatment goals.
