The RN Case Manager Liaison Nurse serves as a critical bridge within the Kaiser Foundation Health Plan of Washington (KFHPW) continuum of care. Based onsite at St. Peter Hospital in Olympia, this position focuses on inpatient discharge planning and follow-up care for patients hospitalized at nearby network facilities. The role exists to ensure optimal patient health outcomes while promoting the appropriate utilization of healthcare resources.
In this capacity, you will work across two settings on a periodic rotating schedule. You will carry a caseload of patients within KFHPW medical centers while simultaneously planning discharges for those in the network. Your primary mission is to establish collaborative care plans with primary care teams, specialty providers, and medical staff. This involves assessing patients' physical, mental, financial, and psychosocial needs to develop patient-centered plans that improve functional status and self-management capabilities. You will act as the liaison between KFHPW and external contracted providers, facilities, and resources, providing feedback to leadership regarding service quality and care delivery.
This is an onsite position at St. Peter Hospital in Olympia. The role operates on a variable Monday through Friday schedule, typically from 10:00 PM to 10:30 AM. The position requires a rotation of every other weekend and alternating holidays. A night shift differential applies to this schedule. Candidates must have prior experience in Emergency Room (ER) or case management work to succeed in this environment.
