The RN Case Manager Liaison Nurse serves as a critical bridge within the Kaiser Foundation Health Plan of Washington (KFHPW) continuum of care. Based onsite at St. Peter Hospital in Olympia, this position focuses on inpatient discharge planning and follow-up care for patients hospitalized at nearby network facilities. The role exists to ensure optimal patient health outcomes while promoting the appropriate utilization of healthcare resources.
In this position, you will work a variable schedule, typically Monday through Friday from 10:00 PM to 10:30 AM, with a rotation of every other weekend and alternating holidays. You will split your time between two settings, managing a caseload of patients within a KFHPW medical center while coordinating discharges for those in the network. As the Liaison Nurse, you will establish collaborative care plans with primary care teams, specialty providers, and medical staff to improve functional status and patient self-management capabilities.
Your day-to-day involves acting as the primary liaison between KFHPW and external contracted providers, facilities, and resources. You will collect data to provide leadership with insights on utilization, cost, quality, and service delivery, while advocating for patient needs and ensuring adherence to medical plans. This role requires navigating complex care scenarios, facilitating referrals to home health, hospice, or long-term care, and supporting patient education to ensure compliance with treatment plans.
