The Infectious Disease, Pulmonary Hypertension, & Neurology Ambulatory Clinical Pharmacist position offers a unique opportunity to provide advanced pharmacy services across multiple highly specialized clinics. This role supports patients with complex infectious, neurologic, and cardiopulmonary conditions requiring high-touch care coordination, specialized therapies, and close clinical monitoring.
In this capacity, you will serve as a key partner to providers, advanced practice providers, and nursing staff. Your day-to-day involves optimizing therapy outcomes, improving medication adherence, and enhancing patient access to life-changing treatments. You will act as a clinical resource regarding emerging therapies and guideline updates while conducting detailed medication reconciliations and therapy reviews to ensure safe, effective, and individualized treatment plans.
