The Lead Case Manager serves as a vital bridge between vulnerable clients and the resources they need to rebuild their lives. In this role, you will arrange, coordinate, monitor, and mentor a team of case managers to ensure all delivered services meet the unique needs of program clients. This position requires a dynamic blend of administrative oversight and field outreach, ensuring that housing stability plans are developed, executed, and followed up on effectively.
You will lead a team through the full spectrum of case management, from reviewing daily referrals and conducting comprehensive assessments to developing support systems and facilitating move-in activities. As a leader, you will review cases with the Program Coordinator, conduct weekly case conferencing, and perform monthly file audits to guarantee accuracy and compliance. The role also involves direct engagement with the community through outreach events and workshops designed to meet the specific needs of veteran households and families.
As service providers to the underprivileged and vulnerable in our community, all employees are considered Mandated Reporters and must report any instance of physical or sexual abuse to proper authorities. Candidates must be able to pass a fingerprint clearance and background check, including criminal history, personal references, and employment verifications. The position requires on-site presence to provide proper direction and oversight to the team.
