About the Role
The Lead Case Manager serves as a vital bridge between vulnerable clients and the resources they need to rebuild their lives. In this role, you will arrange, coordinate, monitor, and mentor a team of case managers to ensure all delivered services meet the unique needs of program clients. This position requires a dynamic blend of administrative oversight and field outreach, ensuring that housing stability plans are developed, executed, and followed up on effectively.
You will lead a team through the full spectrum of case management, from reviewing daily referrals and conducting comprehensive assessments to developing support systems and facilitating move-in activities. As a leader, you will review cases with the Program Coordinator, conduct weekly case conferencing, and perform monthly file audits to guarantee accuracy and compliance. The role also involves direct engagement with the community through outreach events and workshops designed to meet the specific needs of veteran households and families.

Candidates must be able to pass a fingerprint clearance and background check, including criminal history, personal references, and employment verifications. The position requires on-site presence to provide proper direction and oversight to the team.

Requirements
- verifiedAssociate's Degree in Social Work or related field OR combination of education and 2 years experience
- verifiedMinimum 2 years of experience as a case manager
- verifiedAbility to pass fingerprint clearance and background check
- verifiedCompletion of VOALA Case Management training within 6 months of hire
Nice to Have
Bachelor's Degree in Social Work or related field. 3 years of experience working with the target population. Experience in a supervision or lead role. HMIS navigation skills.
