About the Role
As a Case Manager, you will serve as a vital link between exceptional care and the transformative impact on each patient's journey. This role goes beyond a standard job; it positions you to leverage your clinical expertise while contributing to the well-being of individuals in your community. You will manage resources and coordinate patient care from admission to post-discharge, overseeing interdisciplinary plan-of-care decisions.
In this position, you will work closely with an interdisciplinary team to guide treatment plans based on patient needs and preferences. Your day will involve facilitating team conferences, monitoring patient experiences regarding quality and service appropriateness, and ensuring accurate documentation. You will also play a key role in the patient discharge experience, collaborating to establish tentative discharge plans and contingency strategies. By reviewing case management reports and participating in utilization reviews, you will help identify trends and reduce unnecessary acute care transfers, ensuring a seamless transition for patients and their families.

Requirements
- verifiedActive license in RN, SW, OT, PT, ST, or Rehabilitation Counseling
- verifiedCCM or ACM certification required or within two years
- verifiedBachelor's degree in social work, counseling, psychology, or related field
- verifiedAssociate Degree in Nursing with RN licensure (diploma accepted in some states)
- verified2 years rehabilitation experience preferred
Nice to Have
BSN (Bachelor of Science in Nursing) for nursing candidates. Graduate degree for eligible licensed or certified health care professionals. 2 years of rehabilitation experience.
