About the Role
As a Case Manager, you will serve as a vital link between exceptional care and the transformative impact on each patient's journey. Key Responsibilities
- check_circleCoordinate patient care from admission to post-discharge
- check_circleEstablish tentative discharge and contingency plans with interdisciplinary team
- check_circleFacilitate weekly team conferences and coordinate treatment plan modifications
- check_circleComplete case management addendums and required documentation within 48 hours of admission
- check_circleReview case management reports and Key Care Indicators to plan appropriate actions
- check_circleParticipate in utilization review process including data collection and trend resolution
- check_circleMake appropriate referrals to post-discharge providers and physicians
- check_circleEducate patients and families on rehabilitation and establish communication plans
- check_circleAssist patients with timely resource procurement to avoid discharge delays
- check_circleGuide treatment plans based on patient needs and preferences
- check_circleEnsure accuracy of discharge and payor-related information in patient records
- check_circleMonitor patient experience regarding quality, timeliness, and payor expectations
Requirements
- verifiedActive license in RN, SW, OT, PT, ST, or Rehabilitation Counseling
- verifiedBachelor's Degree in social work, counseling, psychology, or related human services field
- verifiedAssociate Degree in Nursing with RN licensure (diploma accepted in specific states)
- verifiedCCM® or ACM™ certification required or obtained within two years
- verified2 years of rehabilitation experience preferred
Nice to Have
BSN (Bachelor of Science in Nursing) for nursing candidates. Graduate degree for eligible licensed or certified health care professionals. 2 years of rehabilitation experience.
