In this role, you will leverage your clinical expertise to coordinate patient care from admission to post-discharge. You will guide treatment plans based on patient needs and preferences, facilitate interdisciplinary team conferences, and oversee plan-of-care decisions. Your day-to-day involves monitoring the patient experience, ensuring regulatory compliance, and collaborating with families to establish communication plans and discharge strategies. BSN (Bachelor of Science in Nursing) for nursing candidates. Graduate degree for eligible licensed or certified health care professionals. 2 years of rehabilitation experience.
Skills: CCM, Acm, RN, SW, Ot, PT, ST, Utilization Review, Case Management.
Education: Bachelor's Degree in social work, counseling, psychology or related human services field; Minimum of an Associate Degree in Nursing for Nursing professionals; Graduate degree preferred for eligible licensed or certified health care professionals.
