In this role, you will serve as a bridge between patients, families, and the interdisciplinary team. Your day will involve managing resources and coordinating patient care from admission to post-discharge, ensuring that every step of the journey is handled with precision and compassion. You will guide treatment plans based on patient needs and preferences, facilitate weekly team conferences, and oversee interdisciplinary plan-of-care decisions. From conducting initial assessments within 48 hours of admission to collaborating on discharge planning and contingency strategies, you will be instrumental in optimizing the patient experience and ensuring quality, timeliness, and service appropriateness.
