Key Responsibilities
- check_circleCreate and maintain a safe, supportive environment for clients
- check_circleCollaborate with the Lead Case Manager to address client needs
- check_circleCo-facilitate group sessions when appropriate
- check_circleConduct bi-weekly, in-person case management meetings
- check_circlePartner with external agencies to coordinate care and treatment plans
- check_circlePresent client psychosocial needs in interdisciplinary meetings
- check_circleKeep accurate, thorough case documentation and reports
- check_circleElevate incidents or grievances promptly to the Program Manager
Requirements
- verifiedHigh school diploma required
- verifiedBachelor's degree in Human Services strongly preferred
- verifiedMinimum 5 years relevant experience
- verifiedLSWA licensure required for candidates with a BSW
- verifiedStrong knowledge of D.C. social services and compliance standards
- verifiedProficiency in MS Word & Excel
