As a Case Manager under the supervision of the Director of Social Service, you will be instrumental in providing comprehensive case management and housing support to our clients. This role exists to ensure compliance with program parameters while guiding families toward independence. You will serve as a vital advocate, conducting needs assessments, intakes, and home visits to understand the unique challenges our clients face. Your day will involve bi-weekly interviews, developing service plans, and coordinating workshops to support the general client population. You will also manage critical relocation services, assisting Head of Households with Section 8, NYCHA, and homeless housing applications, while facilitating apartment inspections and lease signings. Beyond direct client interaction, you will maintain detailed records, provide crisis intervention, and educate participants on housekeeping, safety, and landlord-tenant relations.
