Under the supervision of the Director of Social Service, the Case Manager serves as a vital advocate for families navigating complex housing and social service systems. This role exists to ensure compliance with program parameters while providing essential case management and housing support. The day-to-day involves conducting needs assessments, intakes, and home visits, as well as holding bi-weekly interviews to monitor client progress. You will coordinate workshops, assist in developing service plans aimed at restoring family independence, and provide on-site crisis intervention. A significant portion of the role focuses on relocation services, guiding Head of Households through Section 8, NYCHA, and homeless housing applications, coordinating with landlords and brokers, and ensuring successful lease signings and move-outs.
