As a Case Manager at Encompass Health, you will serve as a vital link between exceptional care and the transformative impact on each patient's journey. In this position, you will manage resources and coordinate patient care from admission to post-discharge, overseeing interdisciplinary plan-of-care decisions. You will work closely with an interdisciplinary team to guide treatment plans based on patient needs and preferences, establish discharge plans, and facilitate family conferences. Your day-to-day impact involves monitoring patient experiences, ensuring regulatory compliance, and assisting patients with timely resource planning to avoid discharge delays. Education: Bachelor's Degree in social work, counseling, psychology or related human services field; Minimum of an Associate Degree in Nursing for Nursing discipline; Graduate degree preferred for eligible licensed or certified health care professionals.
