This position serves as a vital bridge between participants and essential community resources. You will guide individuals through the complexities of program enrollment, ensuring they receive the support they need to succeed. From educational and vocational training to medical care, mental health resources, and legal assistance, your role is to help customers access the services that align with their unique goals.
The day-to-day involves cultivating strong professional relationships and monitoring participant progress. You will collaborate closely with customers to set and accomplish program goals, while also identifying and communicating any barriers that might hinder their engagement. By maintaining accurate case documentation and sharing insights with project staff, you help drive the overall effectiveness of the program.
