As a Dietitian at U.S. Renal Care, you will be an integral part of an interdisciplinary team dedicated to providing excellent care for patients living with kidney disease. Working under the supervision of a RDN Regional Manager, your role is pivotal in shaping the future of kidney care and making a positive impact on the lives of others.
Your work will focus on three core pillars:
Patient Nutrition Using the most current, science-based practices, you will recommend therapeutic diets tailored to patient nutritional needs, preferences, and changes in treatment, always in consultation with the patient's physician. You will review lab results to provide appropriate education on nutritional interventions, covering topics such as fluid and sodium management, potassium control, and CKD-Mineral and Bone Disorder management. Additionally, you will conduct patient assessments, engage in care planning activities, and participate in monthly interdisciplinary patient care conferences.
Teamwork Collaboration is key to your success. You will work closely with the Medical Director, physicians, facility administrators, social workers, nurses, and patient care technicians. This role also involves maintaining positive relationships with area hospitals, skilled nursing facilities, agencies, vendors, and the broader community.
Safety & Quality You will contribute to clinical and operational processes with a focus on improving patient health and achieving target goals for patient outcomes. Part of your responsibility includes maintaining a productive quality assurance program and participating in monthly Quality Assessment and Performance Improvement (QAPI) activities.
