As a Dietitian at U.S. Renal Care, you will be an integral part of an interdisciplinary team dedicated to providing excellent care for patients living with kidney disease. Working under the supervision of a RDN Regional Manager, your role is pivotal in shaping the future of kidney care and making a tangible, positive impact on the lives of others.
Your work centers on patient nutrition, utilizing current, science-based practices to recommend therapeutic diets tailored to individual nutritional needs, preferences, and treatment changes. In consultation with the patient's physician, you will review lab results to provide education on critical topics such as fluid and sodium management, potassium levels, and CKD-Mineral and Bone Disorder. Beyond direct patient care, you will conduct assessments, develop care plans, and actively participate in monthly interdisciplinary patient care conferences.
Collaboration is at the heart of your day-to-day. You will partner with Medical Directors, physicians, facility administrators, social workers, nurses, and patient care technicians to maintain strong relationships with area hospitals, skilled nursing facilities, agencies, vendors, and the broader community. Additionally, you will contribute to clinical and operational processes focused on improving patient health, achieving target outcomes, and maintaining a robust quality assurance program through monthly Quality Assessment and Performance Improvement (QAPI) activities.
