As a Case Manager at Encompass Health, you will serve as a vital link between exceptional care and the transformative impact on each patient's journey. This role goes beyond a standard job; it positions you to leverage your clinical expertise while contributing to the well-being of individuals in your community. You will manage resources and coordinate patient care from admission to post-discharge, overseeing interdisciplinary plan-of-care decisions to ensure truly meaningful outcomes.
In this position, you will work closely with an interdisciplinary team to guide treatment plans based on patient needs and preferences. Your day will involve facilitating team conferences, monitoring patient experiences regarding quality and service appropriateness, and ensuring accurate documentation of discharge and payor-related information. You will also play a key role in identifying trends to reduce acute care transfers and assisting patients with timely resource planning to avoid discharge delays.

BSN (Bachelor of Science in Nursing) for nursing candidates. Graduate degree for eligible licensed or certified health care professionals. 2 years of rehabilitation experience.
Skills: CCM, Acm, RN, SW, Ot, PT, ST, Utilization Review, Case Management.
Education: Bachelor's Degree in social work, counseling, psychology or related human services field; Minimum of an Associate Degree in Nursing for Nursing professionals; Graduate degree preferred for eligible licensed or certified health care professionals.
