The Division of Developmental Disabilities (DDD) is seeking an experienced and highly motivated Case Manager 2 to join our team. DDD provides individuals with developmental disabilities and their families with flexible, high-quality, member-driven services and supports. These services empower individuals to exercise their rights and responsibilities regarding independent decision-making and community engagement.
As a Case Manager 2, you will be part of an amazing team committed to service, community, and teamwork. Your day will involve driving to onsite visits at members' homes, schools, and program sites to build relationships and discuss service needs. You will develop, review, and monitor Person-Centered Service Plans (PCSP) while documenting individual progress and facilitating meetings to assist members and families with their choices. Additionally, you will collaborate with community agencies to provide support during crises and maintain comprehensive case records.
